Remote Case Management Nurse Jobs

Case management (or care management) RNs coordinate care for high-cost, high-complexity members — typically by phone or video, with periodic chart reviews. Most roles are 100% remote with predictable Monday-to-Friday hours. Common specialties: maternity, oncology, behavioral health, complex chronic, transitions of care. Below: every RN case management role on RemoteRN today.

Frequently asked

What's the difference between case management and utilization review?+

UR decides whether a service is medically necessary; case management coordinates the member's longitudinal care plan after that decision. Same skillset, different focus. Some roles blend both.

Do I need a CCM certification?+

Not for entry — most plans hire RNs without it and pay for the certification at year 1 or 2. CCM (Certified Case Manager) typically adds $5-10K to salary.

Is this telephonic-only?+

Most are. Some health plans use video for high-acuity members. A small subset (LTSS assessments, post-discharge transitions) require occasional in-home visits — those are flagged in the role description.
