RN Field Case Manager - Columbus, OH Area ONLY

Optum Home & Community Care is seeking an RN Field Case Manager for the Columbus, OH area. The role involves working with patients primarily in their home setting as part of an interdisciplinary care team including a Nurse Practitioner, Behavioral Health Advocate, and Care Navigator. Responsibilities include assessing health status, developing and implementing care plans, coordinating care, and providing patient education. The position requires travel within 60 miles of residence and a valid driver's license. Requires current unrestricted RN license in Ohio and 2+ years of relevant experience.

Key responsibilities

  • Assess health status of members within scope of licensure
  • Establish goals to meet identified health care needs
  • Plan, implement, and evaluate responses to the plan of care
  • Collaborate with multidisciplinary team to address barriers to care
  • Consult with PCP, specialists, and other healthcare professionals
  • Assess patient needs for community resources and make referrals
  • Facilitate patient transitions between care settings
  • Document in electronic medical record
  • Provide counseling and education to patients and families
  • Participate in quality initiatives and team meetings

What you bring

  • Current unrestricted RN license in Ohio
  • 2+ years relevant experience
  • Experience assessing patients with complex behavioral, social, or functional needs
  • Ability to work with diverse care teams in non-clinical settings
  • Proficient with electronic medical records
  • Valid driver's license and reliable transportation
