Job description
Cityblock Health is looking for an RN Care Manager to join our Baltimore team. This is a hybrid role with remote work and community-based visits. You will manage a panel of members with complex needs, coordinate care across providers, and support members in achieving their health goals. Responsibilities include conducting comprehensive assessments, developing care plans, and collaborating with interdisciplinary teams. Requires an active RN license in Maryland, 2+ years of experience in care management or community health, and strong communication skills. BSN preferred. Salary range: $75,000 - $95,000.
