Case Management RN

Oscar Health is hiring a Case Management RN to join the Clinical Concierge Team. Responsibilities include educating members on improving health outcomes, assisting with transitions from care settings, coordinating care across inpatient, outpatient, post-acute, ER, and home care settings, reaching out to members to develop care plans, communicating with hospital case managers for discharge planning, and communicating with members by phone or secure messaging. Requires active unrestricted RN licensure (compact multistate or specific state), 2+ years clinical experience including hospital, outpatient, or community-based care management, and 1+ years in care coordination and navigation. BSN, Milliman Guidelines knowledge, CCM certification, and behavioral health experience are bonus points. Remote position open to candidates in Arizona, Florida, Georgia, or Texas. Base pay $32.60-$42.79 per hour.
