RN Care Manager - Primarily remote - Greensboro, NC

Cityblock Health is seeking an RN Care Manager to support members in Greensboro, NC. This role is primarily remote but requires occasional in-person visits to members' homes for assessments and care coordination. You will work with a multidisciplinary team to manage a panel of members with complex needs, including chronic conditions and behavioral health. Responsibilities include telephonic and video outreach, care planning, medication management, and connecting members to community resources. Requires an active RN license in North Carolina, 2+ years of clinical experience, and comfort with technology. BSN preferred. Salary range: $75,000 - $95,000.
