Job description
The Field Case Manager RN works with patients primarily in their home setting as part of a care team including a Nurse Practitioner, Behavioral Health Advocate, and Care Navigator. Responsibilities include assessing health status, developing and implementing plans of care, coordinating with multidisciplinary team, consulting with PCPs and specialists, facilitating transitions of care, documenting in EMR, and providing patient/family education. Requires current unrestricted RN license in Alabama, 2+ years relevant experience, ability to travel 100% within 60 miles of residence, valid driver's license, and reliable transportation. Preferred: field-based and case management experience.
Key responsibilities
- Assess health status of members within scope of licensure
- Establish goals and plan of care
- Collaborate with multidisciplinary team
