This hybrid role supports Medicare Advantage members throughout Bergen County, combining in-home visits with telehealth care. Korean and English fluency is essential to building meaningful relationships with Korean-speaking members and caregivers, supporting clear communication, and improving outcomes for medically complex and vulnerable senior populations.
We are seeking a compassionate and driven Nurse Practitioner who is bilingual in Korean and English to join our growing Clover Home Care Team in New Jersey.
What You'll Do
- Conduct in-home and telehealth visits for Medicare Advantage members.
- Provide care and patient education in Korean and English to support effective communication with members and caregivers.
- Perform comprehensive assessments, medication reviews, and chronic disease management.
- Develop and implement individualized care plans that address both clinical and social determinants of health.
- Identify barriers to care, including fall risks, food insecurity, caregiver challenges, and gaps in treatment adherence.
- Collaborate with physicians, care coordinators, social workers, and other interdisciplinary team members to deliver holistic, member-centered care.
- Leverage Clover Assistant and EMR technology to support proactive care management and close gaps in care.
- Monitor health status and intervene early to help prevent avoidable emergency department visits and hospitalizations.
- Educate members and caregivers on disease management, preventive care, and treatment plans.
Qualifications
- Active Nurse Practitioner (NP) license in New Jersey or ability to obtain licensure.
- National board certification as a Nurse Practitioner.
- Active DEA and New Jersey CDS registration, or ability to obtain.
- Fluency in Korean and English, with the ability to conduct clinical assessments, explain treatment plans, and provide patient and caregiver education in both languages.
- Experience caring for adult or geriatric populations in primary care, complex care, chronic care management, transitional care, home-based care, or value-based care settings.
- Strong clinical assessment and chronic disease management skills.
- Passion for serving vulnerable senior populations.
- Comfortable utilizing technology and telehealth platforms to enhance patient care.
- Excellent communication and collaboration skills.
Preferred Qualifications
- Home care, home health, home-based primary care, palliative care, geriatrics, or population health experience.
- Experience caring for Medicare Advantage members.
- Prior experience in value-based care models.
- Experience managing medically complex and high-risk patient populations.
- Experience providing culturally responsive care to Korean-speaking patients and families.
A reasonable estimate of the base salary range for this role is
$155,000—$165,000 USD
