Job description
As a Clinical Lead NP, you provide longitudinal care to patients with CHF, CKD/ESKD, and other chronic conditions. You meet patients in their homes and via telehealth, using advanced analytics and interdisciplinary collaboration to keep patients out of the hospital and slow disease progression. You manage a dedicated panel, blending high-touch clinical expertise with a whole-person approach. Requires active NP license in state of practice, 2+ years NP experience, and ability to travel within assigned region. Salary $105k-$130k.
Key responsibilities
