Job description

Provide In-Home Health Evaluations for health plan members, including medication and medical history review, physical evaluation, and diagnostic tests as ordered. Use iPad and clinical workflow. No prescribing or altering treatment. Travel locally within licensed states. Conduct virtual visits as needed. Per diem role with hourly base rate plus incentives.

Key responsibilities

  • Conduct in-home health evaluations (45-minute visits)
  • Review medication and medical history

What you bring

  • Active unrestricted NP or PA license in Illinois
  • Board certification
  • Multi-state licenses preferred
  • Comfortable working in diverse home environments
