Job description
Provide In-Home Health Evaluations for health plan members in their homes. Conduct 45-minute visits including medication and medical history review, physical evaluation, and diagnostic tests (e.g., diabetic eye exams, spirometry) as ordered. Use iPad and clinical workflow. No prescribing, lab orders, or treatment changes. Travel locally and within licensed states; conduct virtual visits as needed. Full-time role with benefits including health/dental/vision insurance, PTO, CEU allowance, 401K, malpractice coverage.
Key responsibilities

What you bring
