Conduct in-home health assessments for Medicare and Medicaid members in the Kennewick, WA area. Perform comprehensive health evaluations, review medical history, identify gaps in care, and coordinate with primary care providers. Flexible per diem schedule. Must have active NP or PA license in Washington state.
