Job description
Provide In-Home Health Evaluations for health plan members in their homes. Visits last about 45 minutes and include medication and medical history review, physical evaluation, and optional diagnostic tests (diabetic eye exams, spirometry, etc.) as ordered by the health plan. Use an iPad and clinical workflow; no prescribing, lab orders, or treatment alterations. Travel locally within licensed states; conduct virtual visits as needed. Per diem role with hourly base rate plus per-visit incentives. Requires active unrestricted license in coverage area and board certification.
Key responsibilities
- Conduct in-home health evaluations including medication review, physical exam, and diagnostic tests
- Use iPad and clinical workflow for each evaluation
- Travel locally and within licensed states
- Conduct virtual visits as needed
- Provide all Signify Health services including diagnostic and preventive services
