Job description
Provide In-Home Health Evaluations, engaging with people in the comfort of their own homes. The visit lasts about 45 minutes and includes medication and medical history review, physical evaluation, and additional diagnostic tests if ordered. Use an iPad and clinical workflow. No prescribing, lab orders, or altering treatment regimens. Travel locally within licensed states. Conduct virtual visits as needed. Provide all Signify Health services including Diagnostic & Preventive Services (spirometry, DEE, bone density screening).
Key responsibilities
- Conduct in-home health evaluations (45-minute visits)
- Perform medication and medical history review, physical evaluation
- Administer diagnostic tests as ordered (e.g., diabetic eye exams, spirometry)
- Use iPad and clinical workflow for each evaluation
- Travel locally within licensed states

What you bring
- Active unrestricted NP or PA license in Rhode Island
- Board certification
- Comfortable working in diverse home environments
- Ability to travel locally
