Job description
Field RN Case Manager for Senior Community Care team, visiting members primarily in nursing homes. Collaborate with interdisciplinary care team, triage member needs, assist with transitions in care, ensure accurate documentation, medication reconciliation, nursing rounds, and risk assessment. Monday-Friday 8AM-5PM, no nights, weekends, or holidays. Requires active unrestricted Texas RN license, 1+ years clinical experience, BLS certification, ability to lift 30 lbs, and reliable transportation.
Key responsibilities
- Collaborate with interdisciplinary care team on changes in condition and care coordination
- Triage member needs to appropriate care team member and level of care
- Assist with transitions in care (outpatient and inpatient)
- Ensure complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessment
What you bring
- Active unrestricted Texas RN license
- 1+ years clinical experience
- BLS certification
- Ability to lift 30 lbs and perform physical assessment
- Reliable transportation
