Field Registered Nurse Case Manager, Optum Care at Home - Pueblo Counties, CO

Job description

The Registered Nurse in the Optum Care at Home program performs in-home visits, telephonic outreach, and interdisciplinary team activities. Responsibilities include assessing health status, developing and implementing care plans, coordinating care, and educating patients and families. The role requires daily travel to patient homes within El Paso and Pueblo Counties. Requires current unrestricted RN licensure in Colorado, 2+ years RN experience, 2+ years in long-term care, home health, hospice, public health or assisted living, and 1+ years clinical or case management experience. Ability to travel up to 75% of the time.

Key responsibilities

  • Assess health status of members during in-home visits
  • Establish goals and develop plan of care
  • Collaborate with multidisciplinary team
  • Facilitate transitions between care settings
  • Document in electronic medical record
  • Provide counseling and education to patients and families

What you bring

  • Current unrestricted RN license in Colorado
  • 2+ years RN experience
  • 2+ years in long-term care, home health, hospice, public health or assisted living
  • 1+ years clinical or case management experience
  • Computer literate
  • Valid driver's license and reliable transportation
