CVS Health is hiring a per diem Nurse Practitioner or Physician Assistant to conduct in-home health assessments for Medicare Advantage members in Jasper, IN. This is a field-based role where you travel to members' homes to perform comprehensive health risk assessments, including physical exams, medication review, and coordination of care. Flexible scheduling, no on-call or weekends required. Must have an active NP or PA license in Indiana.
