Field Registered Nurse Case Manager - Optum Care at Home, Oklahoma City Metro area

Job description

The Registered Nurse in the Optum at Home program works with patients primarily in their home setting as part of a care team including a Nurse Practitioner, Behavioral Health Advocate, and Care Navigator. Responsibilities include assessing health status, developing and implementing care plans, coordinating care, and providing patient education. The role involves daily travel to patient homes within the Oklahoma City Metro area. Requires current unrestricted RN licensure in Oklahoma and 2+ years of RN experience. Preferred qualifications include home health and geriatric experience. Salary range: $60,200 - $107,400 annually.

Key responsibilities

  • Assess health status of members within scope of licensure
  • Establish goals to meet identified health care needs
  • Plan, implement, and evaluate responses to the plan of care
  • Collaborate with multidisciplinary team to address medical, functional, and social barriers
  • Consult with PCP, specialists, and other healthcare professionals
  • Assess needs for community resources and make referrals
  • Facilitate patient transitions within and between care settings
  • Document in electronic medical record
  • Provide counseling and education to patients and families
  • Participate in quality initiatives and multidisciplinary team meetings

What you bring

  • Current unrestricted RN license in Oklahoma
  • 2+ years RN experience
  • Experience assessing patients with complex behavioral, social, and/or functional needs
  • Computer skills including EMR
  • Ability to work with diverse care teams in non-clinical settings
  • Home health experience (preferred)
  • Geriatric experience (preferred)
