Field Nurse Practitioner (Dallas, TX)

Job description

Provides screening, preventive primary care and medical care services to members primarily in non-clinical settings including in-home, community, nursing facilities, and pop-up clinics. Performs comprehensive evaluations, addresses chronic and acute complaints, orders diagnostics, creates care plans, and coordinates post-discharge care. Drives up to 120 miles daily within assigned region. Requires active FNP license in Texas, DEA license, BLS, and valid driver's license. At least 1 year NP experience.

Key responsibilities

  • Provide general medical care and care coordination to various member populations
  • Perform comprehensive evaluations including history and physical exams
  • Address chronic and acute primary care complaints
  • Order diagnostic tests and create medical plans of care
  • Perform face-to-face visits in homes, skilled nursing facilities, and public locations
  • Drive up to 120 miles daily within assigned region
  • Participate in community-based pop-up clinics
  • Perform on-call coverage for urgent lab results and pharmacy inquiries

What you bring

  • Active and unrestricted FNP license in Texas
  • National certification (AANP or ANCC)
  • DEA license
  • BLS certification
  • Valid driver's license and reliable transportation
  • 1+ year NP experience
  • EMR proficiency
  • Strong communication and time-management skills
