The Field Case Manager RN works with patients primarily in their home setting as part of a care team including a Nurse Practitioner, Behavioral Health Advocate, and Care Navigator. Responsibilities include assessing health status, developing and implementing care plans, coordinating care, and providing patient education. The role involves in-home visits, telephonic outreach, and interdisciplinary team activities. Requires current unrestricted RN license in Alabama, 2+ years relevant experience, and ability to travel 100% within 60 miles of residence.

Key responsibilities

  • Assess health status of members within scope of licensure
  • Establish goals to meet identified health care needs
  • Plan, implement and evaluate responses to plan of care
  • Collaborate with multidisciplinary team to address barriers to care
  • Consult with PCP, specialists, and other health care professionals
  • Assess patient needs for community resources and make referrals
  • Facilitate transitions within and between health care settings
  • Document in electronic medical record
  • Provide counseling and education to patients and families
  • Participate in quality initiatives and team meetings

What you bring

  • Current unrestricted RN license in Alabama
  • 2+ years relevant experience
  • Experience with complex behavioral, social, and functional needs
  • Ability to work with diverse care teams in non-clinical settings
  • Proficient with electronic medical records
  • Valid driver's license and reliable transportation
