Job description

Support Fidelis LTSS member population as a Senior Care Manager. Assess, plan, and implement complex care management activities to drive quality, cost-effective healthcare outcomes. Develop personalized care/service plans, address member issues, and educate members and their families/caregivers on services and benefits.

Day-to-day work includes collaborating with providers, specialists, and community resources to address unmet needs; coordinating between members, caregivers, and care teams to ensure person-centered care; and monitoring care plans and member status, revising plans as conditions change. You'll review member data to identify trends, evaluate referrals and intake assessments, and conduct telephonic, digital, home, or other site outreach as needed to assess member needs.

Documentation and compliance with state, federal, and third-party payer regulations is core to the role. You'll also facilitate education on disease processes, care gaps, and benefits, and partner with leadership to enhance quality delivery. Senior responsibilities include precepting new clinical hires, coaching them through onboarding, completing competency checkpoints, and bridging classroom training with field practice within a Managed Care Organization.

Requirements: RN with NY state licensure (compact accepted in addition), nursing degree or BSN, and 4–6 years of related experience. Salary range $75,300–$135,400.

What you bring
