Job description
Cityblock Health is seeking an RN Care Manager to support members in Fayetteville, NC. This hybrid role is primarily remote with occasional in-person meetings at community locations (not clinical facilities). The Care Manager will coordinate care for complex members, conduct telephonic outreach, develop care plans, and collaborate with interdisciplinary teams. Requires active RN license in NC, 2+ years of experience in case management or community health, and strong communication skills. Salary range: $75,000–$95,000.
