Job description
Cityblock Health is seeking a Remote RN Care Manager for North Carolina. This role involves providing care management to members via telehealth and telephonic outreach, coordinating care, and supporting members with chronic conditions and behavioral health needs. The RN will work collaboratively with an interdisciplinary team to develop care plans, address social determinants of health, and ensure members receive appropriate services. Requires an active RN license in North Carolina, 2+ years of clinical experience, and experience in care management or community health. Salary range $75,000-$95,000.
Key responsibilities
- Provide telephonic and virtual care management to members
- Develop and implement individualized care plans
- Coordinate with interdisciplinary team including social workers and community health workers
- Address social determinants of health and connect members to resources
- Monitor member progress and adjust care plans as needed
