Resource Registered Nurse (RN) – Population Health

Cityblock Health is seeking a Resource Registered Nurse (RN) to join our Population Health team. In this role, you will support complex care management for members with chronic conditions and social needs. Responsibilities include telephonic outreach, care coordination, medication management, and collaboration with interdisciplinary teams. You will use clinical judgment to identify gaps in care and connect members to appropriate resources. This is a fully remote position, but candidates must hold an active Ohio RN license or a compact RN license eligible in Ohio. No in-person patient care is required.
