Nurse Practitioner (Hudson County, NJ)

Job description

Conduct comprehensive in-home health assessments for Medicare Advantage members. Perform post-discharge follow-up visits to reduce hospital readmissions. Collaborate with interdisciplinary teams using data-driven clinical tools. Ideal for NPs proactive in transitional care, confident using technology and clinical data. Requires active NJ RN license, APN certification, master's degree or post-master's certificate in advanced clinical practice, minimum 1 year NP experience in clinical setting, active DEA and CDS registration, current BLS certification. Home care or house call experience preferred. Must be located in or near Hudson County, NJ and able to travel locally for home visits.

What you bring

  • Current Basic Life Support (BLS) certification
