Job description

Provide oversight and guidance to individuals enrolled in the Wisconsin IRIS Self-Directed Personal Care (SDPC) option. This remote, home-based role involves regularly scheduled visits to members' homes. Responsibilities include administering the Wisconsin Personal Care Screening Tool, creating person-centered plans of care, providing personal care oversight, education, and training for participants and care providers, and completing required documentation and follow-up. Requires active Wisconsin RN license, 2+ years nursing experience with at least 1 year serving target groups (adults with disabilities or older adults), valid driver's license, and reliable transportation.

Key responsibilities

  • Administer Wisconsin Personal Care Screening Tool and addendums
  • Document assessments and complete oversight visits and calls
  • Develop individual plans of care and obtain physician orders
  • Submit prior authorization for personal care services
  • Provide personal care training to participants or care providers
  • Complete collateral contacts with IRIS Consultants and physicians

What you bring

  • Active Wisconsin RN license
  • 2+ years nursing experience
  • 1+ year experience with target groups (disabilities or older adults)
  • Valid driver's license and reliable transportation
  • Database operation and data entry
  • Teaching and mentoring skills
  • Microsoft Office proficiency
