Job description
The Nurse Case Manager 1 is responsible for performing telephonic case management within the scope of licensure for members with behavioral health and substance abuse disorders. This role involves using clinical judgment to assess member needs, develop care plans, coordinate services, and monitor outcomes. The position is 100% virtual, requiring an active, unrestricted RN license in a compact state. Requires 2+ years of clinical experience, preferably in behavioral health. Salary range: $60,000 - $80,000.
Key responsibilities
- Conduct telephonic assessments to identify member needs
- Develop and implement individualized care plans
- Coordinate with providers, members, and families
- Monitor member progress and adjust plans as needed
- Document all interactions in the case management system
