Mom and Baby Care Manager - RN

The Mom and Baby Care Manager is responsible for assessing, planning, implementing, and coordinating care management activities with members to evaluate medical and psychosocial needs. This role works with pregnant and postpartum members and their infants to ensure access to necessary services and resources. The position is remote but requires residence in Nevada. Duties include conducting comprehensive assessments, developing care plans, coordinating with providers, and monitoring outcomes. Requires an active RN license in Nevada and 2+ years of clinical experience, preferably in maternal-child health or case management.
