IRIS Self-Directed Personal Care (RN) (Dane County, WI)

Job description

Provide oversight and guidance to individuals enrolled in the Wisconsin IRIS Self-Directed Personal Care (SDPC) option. Conduct in-home assessments using the Wisconsin Personal Care Screening Tool, develop person-centered plans of care, provide training and education to participants and care providers, and ensure compliance with Department of Health Services policies. This is a remote, home-based role with regularly scheduled visits to members' homes in Dane County, WI. Requires active Wisconsin RN license, 2+ years nursing experience, and reliable transportation.

Key responsibilities

  • Administer Wisconsin Personal Care Screening Tool and addendums
  • Develop and revise individual plans of care
  • Provide personal care oversight and training to participants and care providers
  • Submit prior authorization requests for personal care services
  • Complete collateral contacts with IRIS Consultants and physicians
  • Document assessments and oversight visits as required

What you bring

  • Active Wisconsin RN license
  • 2+ years nursing experience
  • Experience serving adults with disabilities or older adults
  • Valid driver's license and reliable transportation
  • Database operation and data entry
  • Teaching and mentoring skills
  • Microsoft Office proficiency
