The Clinical Care Manager is responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical and psychosocial needs of the member. This role works in a hybrid model (remote and in-office) and requires an active, unrestricted RN license in Massachusetts. The position involves telephonic and virtual member outreach, care coordination, and collaboration with providers. No in-person patient care at a clinical facility.
