Job description
The Clinical Care Manager is responsible for coordinating and managing the care of members with complex medical needs. This hybrid role requires working from the Waltham, MA office 2-3 days per week and remotely the rest. Duties include conducting comprehensive assessments, developing care plans, coordinating with providers and community resources, and monitoring member progress. Requires an active RN license in Massachusetts and 2+ years of clinical experience. BSN preferred. Salary range $70,000-$90,000 plus $5,000 sign-on bonus.
