Hybrid Registered Nurse (RN) Clinical Care Manager - Springfield and surrounding areas - 5K Sign On Bonus

The Clinical Care Manager is responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical and psychosocial needs of the member to facilitate and support the member's overall wellness, develop the member's plan of care, and integrate with the member's primary care physician to ensure quality and cost-effective outcomes. This is a hybrid role requiring travel to member homes and community settings within the Springfield area, with the remainder of work performed from home. Requires an active, unrestricted RN license in the state of Ohio or compact license. 2+ years of clinical experience required. Salary range: $65,000 - $85,000.
