Hybrid Registered Nurse Care Manager - Long Island

The Care Manager is responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical and psychosocial needs of the member. This role involves telephonic and in-person outreach to members in their homes or community settings, but does not involve in-person patient care at a clinical facility. The position is hybrid, requiring some days in the office and some days working from home or in the field. Requires an active, unrestricted RN license in New York and 2+ years of clinical experience. Salary range: $70,000 - $100,000.
