Field Nurse Practitioner (Louisville, KY)

Provides screening, preventive primary care and medical care services to members primarily in non-clinical settings including in-home, community, nursing facilities, and pop-up clinics. Performs comprehensive evaluations, addresses chronic and acute primary care complaints, orders diagnostic tests, creates medical plans of care, provides post-discharge coordination, and performs face-to-face visits in various settings. Drives up to 120 miles daily within assigned region. Requires active and unrestricted FNP license in state of practice, DEA license, BLS certification, and valid driver's license. At least 1 year NP experience.
