Job description

Optum is seeking a Case Manager RN to work remotely from anywhere within the U.S. This role involves assessing and interpreting customer needs, identifying solutions, solving moderately complex problems, and providing guidance to others. The RN will conduct comprehensive assessments, develop care plans, implement interventions, and evaluate effectiveness. Responsibilities include reviewing health records, interviewing consumers, identifying health risks, coordinating care, and documenting in appropriate systems. Requires current unrestricted RN licensure in applicable state and undergraduate degree or equivalent experience. The role is part of a team focused on improving health outcomes through case management, health education, and treatment decision support.

Key responsibilities

  • Assess and interpret customer needs and requirements
  • Conduct comprehensive, holistic, or focused assessments of consumer's health care
  • Develop and modify plan of care based on assessment
  • Implement interventions to achieve goals, including referrals and consumer engagement
  • Evaluate effectiveness of interventions and revise care plans as needed
  • Document assessment information and enter data in appropriate systems

What you bring

  • Current unrestricted RN licensure in applicable state
  • Undergraduate degree or equivalent experience
  • Knowledge of computer functionality and software applications
  • Knowledge of state and federal guidelines (Medicare, Medicaid, commercial)
  • Ability to conduct interviews and assess health literacy
  • Experience with evidence-based guidelines
