Case Manager Registered Nurse - Field in Norfolk, VA

Job description

Field-based Case Manager (RN) for Aetna's Long-Term Services and Supports (LTSS) team in Virginia. Conduct in-person assessments in members' homes (up to 75% travel), develop individualized care plans, coordinate medical, behavioral, and community-based services, monitor progress, and advocate for members. Requires RN with active Virginia license, 3+ years clinical experience, and residence in Norfolk or surrounding areas. Work-from-home flexibility with structured weekday hours (Mon-Fri, 8am-5pm).

Key responsibilities

What you bring

  • Active Virginia RN license
  • 3+ years clinical experience as RN (med surg, behavioral health, acute care)
  • Willingness to travel up to 75%
  • Proficiency with computer systems and keyboarding
  • Effective communication skills
