Case Manager Registered Nurse - Field (Camden County, NJ)

Job description

Integrated Care Management (ICM) Case Manager RN for dual eligible (Medicare/Medicaid) members. Conduct assessments, develop care plans, coordinate with providers and community organizations, and use motivational interviewing to engage members. Travel 25-50% within Camden County, NJ (50-mile radius) for in-home visits. Mileage reimbursed. Requires active NJ RN license, 3+ years clinical practice, reliable transportation. Preferred: CCM, 2+ years care management/discharge planning/home health coordination. BSN preferred. Work from home when not traveling.

Key responsibilities

  • Conduct evaluations of member's needs and benefits using clinical tools
  • Develop proactive care plans to address identified issues and enhance wellness
  • Assess need for referrals to clinical resources and interdisciplinary team members
  • Collaborate with supervisor and stakeholders, present cases at interdisciplinary conferences
  • Utilize case management processes in compliance with regulatory and company policies

What you bring

  • Active and unrestricted RN license in New Jersey
  • 3+ years clinical practice experience
  • Reliable transportation and willingness to travel up to 50%
  • Effective computer skills including MS Office and proprietary applications
  • Excellent analytical, problem-solving, communication, and organizational skills
