LTSS Service Coordinator - RN Clinician (Clark County, IN)

The LTSS Service Coordinator - RN Clinician is responsible for overall management of member's case within the scope of licensure. This field-based role involves traveling to member homes in Clark County, IN to perform telephonic or face-to-face clinical assessments for identification, evaluation, coordination and management of member's needs, including physical health, behavioral health, social services and long term services and supports. The RN develops, monitors, evaluates, and revises the member's care plan, identifies members for high risk complications, coordinates care with the healthcare team, manages members with chronic illnesses, and ensures cost-effective utilization of health benefits. The role also involves providing direction and oversight to non-RN clinicians. Requires a current, active, valid and unrestricted RN license in applicable state(s) and at least 3 years of experience with chronic illnesses or disabilities in a service coordination or case management role.
