Care Manager (RN) - WA

Key responsibilities

• Complete comprehensive assessments of members per regulated timelines
• Develop and implement care coordination plan in collaboration with member, caregiver, physician, and other healthcare professionals
• Conduct telephonic, face-to-face or home visits as required
• Perform ongoing monitoring of care plan to evaluate effectiveness and document interventions
• Maintain ongoing member caseload for regular outreach and management
• Promote integration of services including behavioral health, LTSS, and community resources
• Facilitate interdisciplinary care team meetings and informal collaboration
• Use motivational interviewing and clinical guideposts to educate and support members
• Assess for barriers to care and provide care coordination and assistance
• May provide consultation and resources to peers

What you bring

• Active unrestricted RN license in Washington State
• 2+ years healthcare experience, preferably in care management or medical/behavioral health setting
• Valid driver's license, reliable transportation, and adequate auto insurance
• Understanding of EMR and HIPAA
• Knowledge of community resources
• Proactive, detail-oriented, independent worker
• Excellent time-management, prioritization, problem-solving, and critical-thinking skills
• Strong verbal and written communication skills
• Microsoft Office proficiency
