Job description
Develop, assess, and facilitate complex care management activities for members with primarily physical health needs to drive high-quality, cost-effective outcomes. Evaluate member needs, barriers to care, and social determinants of health, then build personalized care plans aligned with member priorities. Collaborate with providers, specialists, and community resources to address unmet needs and close care gaps. Provide ongoing follow-up and monitoring, revising care plans as member status changes. Coordinate between members, families/caregivers, and provider teams to ensure person-centered care delivery. Connect members to local resources for housing, employment, independent living, foster care, and justice-related services as appropriate. Conduct telephonic, digital, and occasional site-based outreach to assess needs. Document all member interactions and care management activities to ensure compliance with state, federal, and payer regulations. Educate members and families on disease processes, healthcare benefits, and care options. Provide feedback to leadership on opportunities to improve quality and cost-effectiveness. Must reside in Texas and hold an active RN license (Texas or Compact). Requires 2–4 years of related nursing experience. Salary: $56,200–$101,000.
