Job description
The Care Manager Mom and Baby RN is responsible for providing care management services to members in the Mom and Baby program. This role involves telephonic outreach, assessment, care planning, coordination of services, and monitoring of high-risk pregnant and postpartum members and their infants. The RN uses clinical judgment to identify needs, develop individualized care plans, and connect members to community resources. Requires an active, unrestricted RN license in Indiana and 2+ years of clinical experience, preferably in maternal-child health. This is a remote position but candidates must reside in Indiana.
Key responsibilities
- Conduct telephonic assessments of high-risk pregnant and postpartum members
- Develop and implement individualized care plans
- Coordinate with providers, community resources, and internal teams
- Monitor member progress and adjust care plans as needed
- Document all interactions in the care management system
What you bring
- Active Indiana RN license
- 2+ years clinical experience
- Maternal-child health experience preferred
- Strong communication and organizational skills
- Proficiency with electronic health records
