Open position: Home Health Registered Nurse (RN)
Sign-on bonus: $10,000
Shift: Days, Monday–Friday, 80 hours per pay period, with holiday and weekend rotation plus on-call responsibilities.
Home Health Registered Nurse
The Home Health RN provides individualized nursing services in the home environment, conducting comprehensive assessments, developing care plans, delivering skilled nursing care, and evaluating each patient's response to treatment. The nurse helps patients and families manage health conditions safely at home through education, clinical monitoring, and close coordination with providers and the interdisciplinary care team.

Who We Are Seeking
Compassionate, dependable Registered Nurses who demonstrate strong assessment skills, clinical judgment, organization, and communication. Successful candidates are comfortable working independently in the home environment, coordinating care across disciplines, educating patients and families, and traveling to patient homes throughout the service area.
Provides professional nursing care in the home by utilizing all elements of the nursing process. Collects health data, demonstrates knowledge and application of patient assessment skills, performs initial and ongoing assessment of patient and family/caregiver. Analyzes the assessment data to determine nursing diagnosis and develops plan of care. Implements the interventions identified in the plan of care in a safe, timely, and appropriate manner. Evaluates the patient's progress toward attainment of outcomes on an ongoing basis. Completes all required documentation accurately, thoroughly, and on time. Educates and involves patients, families, and caregivers to promote self-care activities. Coordinates care with other disciplines and provides supervision of nursing assistants. Identifies and coordinates community resources and reimbursement resources necessary to support the plan of care. Promotes continuity of care through discharge planning, case management and advocacy.
Must hold an active license to practice nursing in the State of Iowa. The position requires at least an Associate's degree in nursing. RN employees are required to have or obtain a BSN with five years after their start date with UnityPoint Health - Finley Hospital. Within 1 year of hire, need to obtain RN licensure with the States of Wisconsin and Illinois. CPR certification is required. Must have a valid driver's license and proof of personal auto insurance coverage.
