RN Field Case Manager, Mecklenburg Co, NC and Surrounding Areas

In this Care Manager role, you will be an essential element of an Integrated Care Model by relaying pertinent information about member needs and advocating for the best possible care. You will assess, plan, and implement care management interventions individualized for each member, directed toward the most appropriate, least restrictive level of care. Identify and initiate referrals for healthcare and community-based services. Develop and implement care plan interventions throughout the continuum of care as a single point of contact. Communicate with all stakeholders to ensure quality coordinated care. Advocate for members and families. Expect to spend about 50-75% of your time in the field visiting members in their homes or in long-term care facilities. Requires current unrestricted RN licensure, 2+ years clinical experience, MS Office proficiency, reliable transportation, and residence in Mecklenburg County, NC or surrounding areas.
