Job description
Cityblock Health is seeking an RN Case Manager to support members with complex health and social needs. This hybrid role involves working from home and in the community (non-clinical settings) to coordinate care, connect members to resources, and manage chronic conditions. You will collaborate with a multidisciplinary team including social workers and community health partners. Requires active RN license, 2+ years of case management or community health experience, and strong communication skills. BSN preferred. Salary range $75,000-$95,000.
