Job description

Optum Home & Community Care is seeking a Registered Nurse Field Case Manager for the Care at Home program in the greater Washington D.C. area. This field-based role involves spending 50-75% of time visiting members in their homes or long-term care facilities. Responsibilities include assessing, planning, and implementing care management interventions; developing care plans; communicating with stakeholders; advocating for members; identifying resources; documenting in EHR; collaborating with interdisciplinary team; providing advanced care planning; and making outbound/inbound calls. Requires current unrestricted RN license in Washington D.C. (or ability to obtain within 90 days), BLS certification, 2+ years MS Office experience, 1+ years post-acute care experience, 1+ years EMR experience, valid driver's license, and reliable transportation. Preferred: CCM, clinical case management, geriatric population, LTSS, HCBS, field-based experience, care plan creation, complex medical/behavioral needs. Salary range $60,200-$107,400 annually.

What you bring

  • Current unrestricted RN license in Washington D.C.
  • Certified in Basic Life Support
  • 2+ years MS Word, Excel, Outlook
  • 1+ years post-acute care (e.g., long-term care)
  • 1+ years EMR experience
  • Valid driver's license and reliable transportation
  • Field-based capacity in Washington D.C.
  • Reside within 50 miles of Washington, DC
