LTSS Service Coordinator - RN Clinician (La Porte County, IN & Porter County, IN)

The LTSS Service Coordinator - RN Clinician is responsible for overall management of member's case within the scope of licensure. This field-based role involves traveling to member homes or community settings to perform telephonic or face-to-face clinical assessments for identification, evaluation, coordination, and management of member needs, including physical health, behavioral health, social services, and long-term services and supports. The RN develops, monitors, evaluates, and revises care plans, identifies high-risk members, coordinates with healthcare teams, and provides direction to non-RN clinicians. Requires a current, active, valid, and unrestricted RN license in applicable state(s) and at least 3 years of experience with chronic illnesses, co-morbidities, or disabilities in a service coordination or case management role. Travel to worksite and other locations as necessary.
