Job description
The LTSS Service Coordinator-RN Clinician is responsible for assessing members' needs, developing care plans, and coordinating services for members enrolled in long-term services and supports (LTSS) programs. This role involves conducting in-home visits to evaluate the member's environment and needs, collaborating with interdisciplinary teams, and ensuring members receive appropriate community-based services. The position requires an active RN license in the state of practice (AZ, NM, or TX) and at least 2 years of experience in case management, home health, or LTSS. Travel within the assigned region is required. Salary range: $65,000–$85,000.
Key responsibilities
- Conduct comprehensive in-home assessments of members' health and social needs
- Develop and implement individualized care plans
- Coordinate with providers, family members, and community resources
- Monitor member progress and adjust care plans as needed
- Document all interactions and maintain accurate records
