In-Home Nurse Practitioner or Physician Assistant (Per Diem) - Ellendale, ND

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to provide In-Home Health Evaluations. The visit lasts about 45 minutes and includes medication and medical history review, physical evaluation, and optional diagnostic tests. You will use an iPad and clinical workflow; no prescribing or altering treatment regimens. Travel locally and within licensed states, conduct virtual visits as needed. Requires active unrestricted license in coverage area and board certification. Multi-state licenses preferred.
