Job description
Provide In-Home Health Evaluations for health plan members, including medication and medical history review, physical evaluation, and diagnostic tests as ordered. Use iPad and clinical workflow. No prescribing or altering treatment regimens. Flexible travel within licensed states. Per diem role with hourly base rate plus incentives.
Key responsibilities
- Conduct in-home health evaluations (45-minute visits)
- Review medication and medical history
- Perform physical evaluation and diagnostic tests (e.g., diabetic eye exams, spirometry)
- Use iPad for clinical documentation
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
What you bring
- Active unrestricted NP or PA license in Oregon
- Board certification
- Comfortable working in diverse home environments
- Proficient with iPad and clinical workflows
