Job description
CVS Health is seeking a Nurse Practitioner or Physician Assistant to conduct in-home health assessments for Medicare Advantage members in the Springfield area. This is a part-time, field-based role where you will travel to members' homes to perform comprehensive assessments, including physical exams, medication review, and health risk identification. You will work independently and document findings in a mobile system. Must have active NP or PA license, DEA license, and reliable transportation. Bilingual Spanish preferred.
