Job description
Provide In-Home Health Evaluations for health plan members, including medication and medical history review, physical evaluation, and diagnostic tests as ordered. Use iPad and clinical workflow. No prescribing or altering treatment regimens. Travel locally within licensed states. Conduct virtual visits as needed. Part-time, 29.5 hours/week. Pay range $46.03-$99.14/hr.
Key responsibilities
- Conduct in-home health evaluations (45-minute visits)
- Review medication and medical history
- Perform physical evaluation and diagnostic tests (e.g., diabetic eye exams, spirometry)
- Use iPad and clinical workflow
- Travel locally and within licensed states
- Conduct virtual visits as needed
What you bring
- Active unrestricted NP or PA license in Illinois
- Board certification
- Multi-state licenses preferred
